Necessary Forms:
- Once an appointment has been made, you can fill out these forms:
Medical History Form 
Demographic Data Form
- If you do not have access to a printer, the forms can be filled out when you arrive at our office.
- We will need to know your medical history, medications, allergies, surgeries and medical problems you've had in the past.
- Additional forms are required for billing and insurance information.
- Family planning services provided are documented by number, not name. Your privacy is protected.
- We will ask you to name a trusted person with whom we can communicate if the need arises.
